Puppy Palace General Terms, Conditions and Liability Waiver Form

[Client’s Full Name], hereinafter called “customer”, hereby acknowledge and agree to the following terms and conditions related to the services provided by Puppy Palace LLC, hereinafter called “company”:

1. I understand that boarding, daycare, a new environment, location and/or grooming procedures may cause physical or emotional stress to my dog, and I release the company and its employees, partners, contractors and associates from any liability arising from such stress or any resulting injury or illness from any services provided by the company.
2. I certify that my dog is in good health and has received all necessary vaccinations. Any dogs without updated vaccinations WILL NOT be accepted. For the safety of the company, other dogs and all staff no exceptions will be made. I understand that the company reserves the right to refuse service to any dog displaying signs of illness or aggression. I understand that the company reserves the right to refuse any dog for any reason.
3. I understand that all dogs using our boarding, daycare, and training services must be current on all vaccines and MUST provide a copy of records. .
4. I agree to disclose any known medical or behavioral conditions my dog may have, including but not limited to allergies, sensitivities, or aggression towards people or other animals.
5. I authorize the company to seek veterinary care at my expense if my dog becomes injured or ill during grooming, and I am unable to be reached.
6. I understand that while the company takes every precaution to ensure the safety of my dog, accidents may still occur. I agree to hold the company harmless for any injuries or damage(s) that may result from any services provided by the company or any other dogs, staff, employees, associates or partners.
7. I understand that grooming services may involve the use of sharp tools and grooming products that could potentially cause injury to my dog if mishandled. I release the company from any liability related to such incidents.
8. I certify that my dog is not in heat and acknowledge the company will not accept any female dogs in heat for boarding or daycare and if my dog becomes in heat during her stay, I must make arrangements for her to be picked up. A $50 fee will apply every night she is here and in heat.
9. I acknowledge the following late or no-show policies. All dogs must be picked up from grooming by 5:00pm unless otherwise said or you will be charged a $10 fee. All dogs must be picked up from boarding by 10 am the next day or a full daycare daily rate will apply or daycare by 6:00pm or a $10 fee will apply.
10. I acknowledge that all customized training programs or packages require 50% payment at the beginning of or first day of the training session then the remaining balance will be due when you pick up your dog on the last day or end of the program. Group classes or standard packages will be paid in full upon signing up.
11. I acknowledge all cancellations will need to be made 24 hours before the appointment day. If I fail to do so and cancel the same day of the appointment, I will be charged half of the grooming price or for boarding, the first night rate. If I do not call and do not show up for the appointment, I will be charged the full amount any services. In addition, related to grooming services, if I’m 15 or more minutes late, will be charged a $10 late fee. If more than 30 minutes late will need to reschedule and will be charged half the amount of the service.
12. By signing this agreement, you, the customer, agree to relieve the company and its agents, employees, associates and/or partners from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by the company. The pursuit of any veterinary, behavioral, or rehabilitation services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve the company and its agents, employees, associates and/or partners from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used as well as the facility itself and/or other dogs.

I acknowledge that I have read and understood all the terms and conditions outlined in this Terms, Conditions and Liability Form, and I voluntarily agree to abide by them.
